Student Feedback - Questionnaire Form Design

By using a basic OMR template with ten questions, each having four or five responses and a comment box, you have the flexibility and freedom to personalise the questionnaire to your own authority s requirements. You are able to tailor the questions and integrate your own design, colours, logos, capture fields and other boxes, all for a one off design cost of £500.  Should you wish to incorporate additional questions or build the design differently, for example to a double sided form, this is available at an additional charge.

Prices start from just £750 for 5,000 single sided questionnaires printed in three colours.

Questionnaire Scanning

If you stick to the basic OMR template, we can use a standard OMR decode to scan and interpret any data on the form, so no charge will be made.  Should you wish to incorporate additional questions and capture areas, however, a new decode will be required and a charge will be made. DRS will scan the forms on three mutually agreed dates throughout the year, typically once per term.
